MEMBERSHIP CANCELLATION

If you’re sure you’d like to cancel or downgrade your membership at CrossFit 845, you may do so by completing and submitting the form below. This will serve as your 30-day written cancellation/downgrade notice as required by your membership agreement. Your membership will be canceled/downgraded 30 days from the submission of the form below.

This form must be submitted on the 1st of the month in order to not be billed in full for the following month. Submissions made after the 1st of the month will have your next scheduled payment processed as scheduled. (Example, a form submitted on July 2nd, will have the monthly payment billed on August 1st as normal). All payments are non-refundable.

If you decide to reactivate your membership in the future (and we hope you do) membership rates in effect at the time of reactivation will be applicable as membership rates are subject to change.
